Job Description:


Compensation : $140000 - $150000
Job ID : 45507

  • Deliver primary care services to adult patients, including health screenings, diagnostic evaluations, and therapeutic interventions

  • Perform detailed medical assessments and formulate individualized care plans

  • Facilitate coordination of services and specialty referrals in partnership with clinical staff

  • Complete thorough and timely medical records documentation and engage in periodic chart audits

  • Work alongside clinical leadership to enhance care delivery processes and overall service excellence


Job Requirements:


Job Requirements:

  • New York State license, DEA, CSR preferred
  • Masters Degree as a   Nurse Practitioner
  • 2 plus year experience as Nurse Practitioner
