WCS Healthcare Partners is seeking a full-time Registered Nurse to provide case management support and patient care to individuals with Intellectual and Developmental Disabilities (I/DD). This role combines clinical nursing skills and case management through patient health oversight and individualized case planning within a supportive, mission-driven environment.

Compensation : $50 - $55

Job Function:

  • Day Shift.
  • Manage an OPWDD caseload of 18–25 individuals, ages 20 through adulthood, with moderate to profound intellectual and developmental disabilities (IQ 40–70).

  • Travel between Red Hook and Tivoli/Columbia County residential group homes to conduct assessments, monitor health needs, and support staff.

  • Maintain accurate health records and ensure compliance with developmental disabilities nursing standards.

  • Oversee and support medication administration through AMAP-certified staff.

  • Attend medical appointments as needed and collaborate with healthcare providers.

  • Participate in interdisciplinary team planning and care coordination.

 

 


Job Requirements:


Job Requirements:

  • Active Registered Nurse (RN) license in the state of practice.

  • Associate or Bachelor’s degree in Nursing (BSN preferred).

  • Prior experience in case management, utilization review, or care coordination preferred.

  • Proficiency with electronic medical records (EMR) systems.

  • New grads are welcome!
