A Residential Treatment Facility (RTF) serving youth ages 12–18 is seeking a Transition Care Coordinator to support youth and families in treatment and discharge planning.

Compensation : $40 - $43.5

Job Function:

  • Manage a caseload of youth and families, supporting individualized transition and discharge planning.
  • Collaborate with clinical and interdisciplinary teams on treatment planning, admissions, and discharge coordination.
  • Conduct home, school, and community visits to assess readiness and support safe transitions.
  • Escort youth to community appointments, interviews, and services as needed.
  • Engage families to identify strengths, barriers, and supports needed for successful reunification.
  • Coordinate with external systems including education, child welfare, and social service agencies.
  • Facilitate linkage to community resources, supports, and wraparound services.
  • Support family engagement activities including visitation, scheduling, and skill-building.
  • Ensure educational continuity in collaboration with school-based partners.
  • Track and manage transitional funds and ensure compliance with regulatory requirements.
  • Maintain accurate documentation and ensure compliance with applicable guidelines.

Job Requirements:


Job Requirements:

  • Master’s degree in Social Work, Mental Health Counseling, Psychology, or related human services field
  • LMSW or LMHC required
  • Minimum 2 years of experience working with children/adolescents with serious emotional disturbance or in care coordination roles
  • Proficiency with Microsoft Office Suite (Outlook, Word, Excel, PowerPoint)
  • Ability to navigate multiple systems and community resources
  • Flexibility, initiative, and strong time management skills
  • Ability to work both independently and as part of a multidisciplinary team
